Abstract: At 9:30 p.m. on August 6, 2007, a cleaning crew of four employees completed cleaning the equipment in the Kettle Room. As the cleaning crew left the Kettle Room, Employee #1 returned to the ribbon blender to do some additional cleaning on the machine. While Employee #1 was wiping down the ribbon blender with a hand towel, he approached the open discharge gates of the ribbon blender. When Employee #1 began to clean around the open discharge gates with the hand towel, the hand towel got caught in the ribbon of the rotating ribbon blender's auger, pulling Employee # 1's right arm into the machine's auger, and mangling Employee #1's right arm. Members of the cleaning crew in the assembly room heard the screaming of Employee #1 and ran to the Kettle Room to assist Employee #1 and called emergency medical services. The Torrance Fire Dept. and paramedics responded to Michael B's and transported Employee #1 to Harbor UCLA Medical Center. At Harbor UCLA, Employee #1's mangled right arm was amputated at the right elbow.
