Abstract: On September 26, 2006, Employee #1 was heading to repair a malfunctioning dock plate. He approached the dock opening, backing a Crown pallet jack (Model Number 60 PE-27-3, Serial Number 6A115254, weight 1358 lbs) on a sloped surface. According to Employee #1, the malfunctioning dock plate was stuck in a position projecting out. His right middle leg struck the dock plate and became trapped between the plate and the pallet jack. Employee #1 was hospitalized at Long Beach Memorial Medical for a fractured right leg. After the accident, tests revealed that the actual stopping distance of the pallet jack was more than 100 in. The manufacturer's specifications indicated the stopping distance was 34 in. to 52 in. There were no witnesses to this accident. Employee #1 was not aware of the defective pallet jack's brake and indicated that was the cause of the accident.
