Abstract: At approximately 6:15 p.m. on January 17, 2007, Employee #1, a department supervisor, used the friction of the spring-loaded extended pin dragging against the frame of an approximately 6-ft tall Cannon metal portable materials handling cart, Model Number 11-97MN830241000, to hold the shelf in a non-standard position halfway up. The shelf, which Employee #1 had propped up with products on it, fell down; catching his finger in a scissors motion as it fell. The shelf is hinged at the back and can be swung all the way up for easier lower shelf access by moving a spring-loaded pin on the right side of the shelf and then securing it in a hole in the side of the back right side of the frame. When the shelf fell, it severed the fleshy tip of Employee #1's right middle finger in a pinch point on the hinged middle shelf. Employee #1 sustained the amputation of his finger tip and was treated at the hospital but was not admitted.
