Abstract: At 4:00 p.m. on October 31, 2006, Employees #1 and #2 were engaged in renovation work at a job site located in La Habra, California. Their employer was Ace Air Clean Environment, Inc., a general building contractor. Employees #1 and #2 were installing roof trusses on a single story church assembly room. They were using a crane to lift the trusses in place, and scissor lifts to access the trusses for installation of the support bracing. Employee #1 was working from the top of the trusses installing bracing. Employee #2 was in the scissor lift helping to support the trusses. After installing 15 of the 16, A4 trusses that measured approximately 66 ft 5 in. long, it was determined that one truss was missing. The employer skipped the missing truss and continued with the installation of the first of eight required 66 ft 7 in. long, A5 trusses. In place of the missing truss, the employer utilized a JLG 1932E2 Scissor Lift (Serial Number ES19-009) that was raised between the trusses, and tied-off the first A5 truss to the lift, to provide support while securing the next several trusses. At the end of the shift, Employee #2 lowered the scissor lift. As it was descending, the lift toppled, causing the six installed A5 trusses to collapse. An investigation of the accident concluded that one of two scenarios existed. The lift was either not detached from an installed truss, or it had struck an installed truss as it was lowered. Employee #1 fell with the trusses, struck his head, and was pinned under several trusses. Employee #2 managed to stay attached to the scissor lift and was uninjured. Coworkers were able to free Employee #1 and called emergency medical services. The medical responders took Employee #1 to the hospital. He was treated for a head laceration, and released from the hospital within three hours.
