Abstract: On July 20, 2005, and employee and a coworker were preparing to dump a bag of silica into the 34.5-in. tall by 24-in.wide by 42-in. long hopper that tapered down to a 24-in by 12-in. opening into the mix tank. The coworker was operating a forklift with the large bag of silica hanging from the forks. The employee was beginning to place a loading cover on top of the hopper, when he backed up and fell face first into the hopper. The extended left arm went into the opening of the mix tank and struck twice by the two-inch churning paddles. The coworker heard the screaming and saw two legs sticking out of the hopper. The coworker grabbed the legs and pulled the employee from the hopper. The employee was transferred to UCI Medical Center. The employee suffered severe lacerations of the left arm, requiring hospitalization for two months and several reconstructive surgeries.
