Abstract: On February 7, 2005, Employee #1, with Legacy Painting Co., was painting the walls and doors of a parking garage building at a residential construction site. He was on a Werner Type 1 aluminum extension ladder (29 ft working height and 250 lbs. capacity). The ladder was set up against the building roof so that he could paint upper surfaces. The ladder rungs were moist from dew. Employee #1 placed both ladder base safety shoes on a piece of 1-mil-thick plastic sheet. When his feet slipped on a wet rung, both Employee #1 and the ladder fell to an asphalt road. He was hospitalized at West Anaheim Medical Center for a compound fracture of his left leg. The causes of the accident were the wet rungs of the ladder and the slippery material on which the ladder was placed.
