Abstract: On January 29, 2004, Employee #1 was transporting roofing materials from the ground to the roof of a two-story residence using an inclined portable conveyor. He and coworkers set up the conveyor, and then disassembled a two-section aluminum extension ladder to access the different roof levels. The fly section of the ladder was used to access the lower level roof and the base section (with the ladder feet) was placed on the lower level to gain access to the higher roof. After the employees had distributed the roofing materials around the roof, Employee #1 and a coworker were instructed to come down. As Employee #1 descended the ladder, the top of the ladder swung sideways, causing him to fall to the lower roof, roll down to the eave, and then fall again to the ground. He was hospitalized for a fractured left hand. The causal factors of the accident are: failure to use the ladder on a surface that was not flat and level, and failure to secure the top of the ladder into position.
