Abstract: At approximately 6:45 a.m. on April 10, 2003, Employee #1 was operating a Uniloy Blow molding machine, Serial Number 350R2-2637. By looking through the window near the machine control panel, he discovered that a low profile gallon bottle was stuck at the tip of the blow pin just outside the mold. He shut off the machine by activating three switches. He then went to the platform, in front of the die head, and tried to remove the jammed bottle from the blow pin with his right hand. The mold closed on his right hand, amputating his index finger and crushing his middle finger. The employee was transported to the UCI Medical Center where he was hospitalized for 3 days.
