Abstract: At approximately 8:30 p.m. on March 15, 2002, Employee #1 fell from a height of approximately 9 ft from a scaffold while installing red head anchors into a cinder block wall, the scaffold, manufactured by Waco and rented from Paramount scaffold rentals, ran approximately 10 ft along a southern interior wall to a corner and 10 feet along a western wall. Employee #1 was working alone from a scaffold on the ground floor installing the ledger for the second story floor joist. The employer was constructing a 6,000 sq ft second story addition to an existing building on a relatively clean site. Employee #1 fell through the scaffold at the corner intersection and suffered a fracture of the upper left arm in four places. The employee was hospitalized for two weeks after surgery was performed.
