Abstract: At approximately 10:00 a.m. on September 23, Employee #1, a maintenance mechanic, was using a forklift to separate the chassis from the frame of a Ford 3/4-ton flatbed pickup truck, #121. He and a coworker were attempting to adjust the chassis manually when the chassis slipped and his right hand became caught between it and the tire on the pickup. His right index finger was amputated and his middle finger sustained damage. He was taken to Long Beach Memorial Hospital, where he was treated and kept overnight for observation. Employee #1 worked for a junior college and had been in his position full-time for six months,
