Abstract: On September 18, 1997, Employee #1 was operating a double and fold machine, which transfers fabric from a roll onto a rectangular bolt. The machine stalled and he began to make repairs. In the process, Employee #1's left fingers became caught in a chain and sprocket drive, amputating his middle finger. He did unplug and stop the machine before starting to work on it.
