Abstract: At 4:30 p.m. on August 28, 1997, Employee #1 a carpenter in the facilities management trade area of California State University/Long Beach, was using a Class B table saw to make a 1/2 in. by 1/4 in. dado field cut in a piece of plywood. He was applying pressure to the plywood when the piece lost contact with the table saw guide and shifted to the right, causing Employee #1's left index finger to strike the dado blades. Employee #1 went to a hand specialist at Mullikan Medical Center for treatment, but the finger did not respond well to treatment, and the physician was forced to amputate the upper portion of Employee #1's index finger.
