Abstract: At approximately 11:00 p.m. on August 31, 2005, Employee #1 was working as an independent owner-operator of an over-the-road tractor trailer leased by CRST Malone. He arrived at dock Number 5 of warehouse building Number 829 to pick up a load of aluminum coils. A little later, another truck driver saw Employee #1 on top of his load of aluminum kicking at his tarpaulin to cover the load before transport. At approximately 12:00 midnight on August 31, 2005, this same truck driver saw a tarp lying on the concrete pavement beside Employee #1's tractor trailer. At 12:12 a.m. on September 1, 2005, Employee #1 was discovered under this tarp by an Alcoa, Inc., plant protection officer. He was initially conscious and confused, not able to communicate verbally, and bleeding severely from his head and right ear. The plant protection officer immediately called Alcoa's emergency response personnel, who arrived at the accident scene to provide emergency care. Warrick County emergency medical services were called to the accident scene to administer continued care. At 1:06 a.m. Employee #1 was transported by helicopter to Saint Mary's Hospital in Evansville, IN. Employee #1 died at 8:55 p.m. on September 3, 2005, from the skull fractures and subdural and subarachnoid hemorrhages resulting from his fall onto the concrete pavement. Photos taken by an Alcoa, Inc., security officer show the blood where Employee #1 struck the concrete pavement. The point of impact was approximately 4 feet out from the front set of two tires on the rear of the right-hand side of the truck. Employee #1 then moved 3 feet toward the back set of tires on the rear of the right-hand side of the truck.
