Abstract: Employee #1, a fork lift truck operator, had just driven a pallet of stock onto a trailer that he was loading. He positioned the pallet against the right side wall and was backing straight out when the right rear wheel of the fork lift hit a slightly raised portion of the dock plate well, causing the fork truck to bounce. Employee #1 was not wearing a seat restraint and was bounced off his seat with his upper body leaning half way out of the fork lift. As he became off balance, he accidentally hit the gas pedal and turned the steering wheel. He lost control of the fork lift. His upper body was crushed between the wall or the over head door frame (not sure which) and the right rear up-right of the fork lift's protective cage. He managed to stop the fork lift but was trapped in this position. He yelled out in pain and another employee working near by came to assist him by reaching over him to the gear shift lever and drifting the fork lift forward enough to remove Employee #1 had a few lacerations and was treated at a local hospital.
