Abstract: A heavy equipment rigging and moving contractor was hired to delivery and install a new boiler system into the basement of a building owned by a county government. The boiler was in eight separate sections and was being removed from the delivery truck by crane and lowered onto equipment dollies to be pushed by hand into the building. While moving one of the sections, the dolly kicked out, causing the boiler to fall over striking and crushing Employee #1's left foot. Employee #1 was hospitalized and his foot had to be amputated.
