Abstract: On June 6, 2007, Employee #1 was working on a roof approximately 27-30 ft. The employee fell through a hole in the roof on to a gym floor. Employee #1 was air lifted to Roanoke Memorial Hospital suffering from blunt force head and pelvic trauma. The opening of the hole in the roof was unguarded and fall protection equipment was not used. The employee died of his injuries on June 7, 2007.
