Abstract: At approximately 8:30 a.m. on July 8, 1999, Employee #1, a bolting foreman, was walking on the edge of the Q-decking that had just been laid down, observing the area where the next work was going to take place. His foot apparently became caught in one of the cells of the Q-decking and he lost his balance, falling approximately 30 ft to the sub-basement. He was transported to Long Island College Hospital, where he was pronounced dead.
