Abstract: At approximately 9:00 a.m. on September 13, 2006, an employee was removing concrete forms along a floor opening, on the third floor of a building under construction. While working around the unguarded floor opening, the employee fell approximately 18 ft through the opening and landed on the first floor. The employee was not wearing fall protection. The employee was transported to a local trauma center, where he was pronounced dead on arrival.
