Abstract: On June 12, 1995 a concrete truck driver was found in a sand bin. The accident area where the employee was working was out of visual range of the plant operator and other drivers. The employee was drawn into feed at the bottom of a sand bin. He either stepped into the bin to push down sand, or fell in. There was no protection or guarding of the unit which was level to the ground at the feed area, with a short apron in front to prevent loading trucks from running into the opening. The deceased had come into work a little early. He was waiting, along with other truck drivers, to get his truck loaded for his first delivery. He was next to be loaded, when coworkers started looking for him. He was seen earlier, operating a front end loader to fill the sand bin, but no one had seen him after that. This was normal prefill activity for any of the drivers. The operator noted that sand feed was spurting, partially blocked, noted foot prints leading to the sand bin and the employee nowhere to be found. After 1.5 hour, rescue personnel were able to empty out sand around the employee, but needed to cut open the base outlet to free the employee. He was pronounced dead at 10:41 a.m. on site--suffocation from engulfment in sand bin. The feed bin was 10 ft by 10 ft, about 14 ft deep, concrete walls with feed at the base. A vibrator shakes sand down onto a conveyor belt which transfers it to the concrete mixer. The pay loader was adjacent with its motor running and parking brake on. Coworkers speculate that employee had walked into the bin to retrieve a piece of sod and had been sucked in. There were no apparent weather or equipment difficulties on this day. Rescue was called immediately and arrived on site at 8:34. The deceased's head was uncovered by 9:15, but unresponsive. He was freed from the bin by 9:40 a.m. but pronounced dead.
