Abstract: Employee #1 was working with two coworkers on a tube and coupler scaffold. Employee #1 was using an attachable ladder to descend to the next lower level work platform which was approximately 8 ft below the original work platform. Employee #1 fell over the standard guard rail where the employees had accessed the scaffold earlier. This allowed Employee #1 to fall to another lower level. No one actually saw Employee #1 fall and, therefore, the actual elevation could not be determined. The maximum fall was approximately 24-ft 2-in. Employee #1 was killed.
