Abstract: At approximately 1:05 p.m. on January 13, 2004, Employee #1 and a coworker were removing wood from a concrete form. Both Employee #1 and the coworker were located on opposite sides of a 28-foot high wall. Although Employee #1 was wearing a full-body harness and using one positioning hook for fall protection, this particular system would not protect him when moving from one work position to another. As Employee #1 and the coworker were moving to a lower work elevation, approximately 20-feet high, Employee #1 fell. Another coworker witnessed Employee #1 fall and called emergency services. Employee #1 was pronounced dead by the Berks County Coroner.
