Abstract: On March 15, 2008, Employee #1 had just completed unloading a 2,500-pound concrete burial vault, using a knuckle boom attached to a 2008 Ford F550 flatbed truck. The truck became stuck in the snow at the Macville Cemetery, located on Highway 169, in Aitkin County, MN. Employee #1 attempted to use the knuckle boom to lift and push the truck and free it from the snow. Employee #1 was operating the boom from the rear passenger side of the truck and lowered the boom to the ground next to him. As Employee #1 forced the boom into the ground, the truck began to slide. The boom slipped and as it made contact with the truck bed, Employee #1's right hand was caught between the boom and truck. Employee #1's right hand was severed and later reattached. Employee #1 died on April 27, 2008.
