Abstract: On May 6, 2006, Employee #1 was working for a manufacturer of industrial organic chemicals. He was making adjustments on the conveyor belt of a raw vacuum filter. The adjustments were being made following the replacement of the large rubber conveyor belt, a procedure that requires a group lockout. The belt adjustments are known throughout the industry as "tracking the conveyor belt." Tracking the belt is a process in which the tail pulley is tensioned and the supporting rolls are shifted, so that the conveyor belt runs as straight as possible. This reduces or eliminates lateral movement of the belt, which could cause the conveyor belt to jump off the pulleys. As he worked, Employee #1 was caught by the moving conveyor belt and the rotating support roller. His chest was crushed between the rotating tail end support roller and a vacuum conveyor structural support beam. He was killed.
