Abstract: At approximately 10:00 a.m. on October 3, 2005, an employee and a coworker were loading a Caterpillar 938F onto a flatbed trailer that was hooked to a Freightliner truck. The employee was driving the truck. The truck and trailer were positioned on the top of a hill on a dirt road to load the Caterpillar 938F equipment. The employee engaged the air brakes on the truck, which automatically engaged the brakes on the trailer. The employee got out, and the coworker began to move the Caterpillar 938F up the ramps onto the trailer. The coworker noticed the employee running toward the truck and at the same time he noticed the trailer was moving. The coworker lost site of the employee. The truck, trailer, and equipment slid down the hill and jackknifed into the west bank of the hill, came back out into the road and stopped. The coworker got off the equipment, went to the truck and found the employee pinned under the right tie rod of the truck just behind the right front wheel. The coworker checked for a pulse and called emergency medical services. The employee was killed.
