Abstract: On November 25, 2007, Employee #1, the brother of the owner of the company, brought a LP gas heater and empty 20 pound cylinder into the establishment at the owner's request. The LP gas heater was located inside a wooden structure that also housed two cases of fireworks. Employee #1 arrived on site at approximately 11:00 a.m. to start work and around 3:00 p.m. He told a witness he was going to a filling station to fill the LP tank. Employee #1 returned around 3:30 to 4:00 p.m. At approximately 4:45 p.m., he made all connections to the heater. He began trying to light the heater with a regular cigarette lighter. Around 5:00 p.m. the witness heard a loud boom, and saw Employee #1 exit the building engulfed in flames. The flames were also coming from inside the building. Employee #1 died of multi-system organ failure due to burns on 90 percent of his body.
