Abstract: Employee #1 was operating a Vicker Engineering CS-18 core cutter. He placed his thumb on top of the core to pull it out of the machine when the blade fell and severed his thumb. Subsequent investigation revealed that the machine had unexpectedly failed because a pin that secured the blade in its proper position had become dislodged, allowing the blade to fall. Coworkers stated that they had been instructed not to place their fingers on top of the core.
