Abstract: The HF Alkylation Unit of the refinery was under normal operations. Two contractor workers were servicing a temporary clamp box on a special three-way valve. One connection to the valve blew off while more sealant was being added to the box. A vapor mixture of isobutane, propane, and residual hydrofluoric acid was released from a 3-inch line at 300-400 psig. The release quickly ignited, causing a fire. The fire caused heavy damage to an overhead pipe rack and run of wiring. A high-pressure hydrogen line and additional lines failed, causing some explosive popping sounds. Three employees were hospitalized with injuries from the fire.
