Abstract: On June 13, 2002, Employees #1 through #5 were installing roof trusses while standing on the ceiling joists which were covered with plywood. A main beam (constructed of spliced 2 in. by 10 in. boards nailed to either side of half-inch plywood with 2-in. by 4-in. boards on top and bottom) collapsed, injuring all five. Employees #1 through #4 were hospitalized with fractures. Employee #5 was hospitalized with contusions and abrasions.
