Abstract: On June 13, 2013, Employee #1 was reaching through an electric entrance gate attempting to get it open. The entrance automatically opens at 6:00 a.m. every morning and closes at 6:00 p.m. every evening. At 5:59 a.m., Employee #1 stuck his arm through the gate to try and trip the sensor on the gate. At 6:00 a.m., the gate automatically opened and severed Employee #1's right arm below the elbow. Employee #1 was hospitalized due to serious injuries.
