Abstract: On December 5, 2005, Employee #1 was recovering a truck with a traffic attenuator, from a highway construction site. The attenuator could not be winched up into the travel position, because the screws securing the strap to the winch had been sheared off. The attenuator was lifted into the travel position by a group of employees. Once in the travel position, the employees believed the attenuator was locked into place. Employee #1 was killed when he walked under the attenuator to secure the safety chains, and it fell on him.
