Abstract: At approximately 2:00 p.m. on September 22, 2008, Employee #1 was observed staggering in the east hallway of Building Number 310, on the ground floor of the hospital. A coworker noticed that the Employee #1 was having trouble walking and came to his aid. The coworker helped walk Employee #1 to the second floor Emergency Room. At this time, the coworker noticed blood covered by hair on the back of Employee #1's head. The coworker asked what had happened and Employee #1's response was "Your table in the break room moved". The coworker said Employee #1 was very dizzy while walking and stopped several times along the way. Employee #1 checked himself in at the ER desk at approximately 2:10 p.m. and sat in the waiting room with the coworker. The coworker said Employee #1 could remember it was Monday but could not remember the President's name. The safety manager stated the victim was seen by a physician in the ER. Employee #1 was taken by life-flight to another hospital at approximately 3:30 p.m. where he received treatment. A coworker talked with Employee #1 earlier in the morning the day of the incident. Employee #1 had complained to the coworker of being dizzy and had a bad weekend with dizzy spells and sleepless nights. Employee #1 was taking daily medication to prevent heart attacks, strokes, and blood clots in veins and arteries. Employee #1 died at approximately 11:20 p.m. on September 22, 2008. Preliminary results indicate Employee #1 died as a result of sub-dermal hematoma.
