Abstract: Employee #1 was preparing to install a new pipe to an existing materials handling system. He was using an electric welder and a cutting rod to make a hole in the pipe when the rod became stuck. There was a platform with a guardrail that extended partially behind him. The surface on which he was standing was approximately 29 in. higher than the platform, and 10 in. lower than the guardrail. Employee #1 was trying to work the rod loose when he apparently lost his balance and fell backward. He struck the top of the guardrail at the corner of the platform and rolled off, falling 26 ft 3 in. onto compacted dirt. He sustained injuries that required hospitalization. Employee #1 was not using any form of fall protection at the time of the accident.
