Abstract: On October 31, 1995, Employee #1, of Quality Transportation Services, was operating a White Freight Liner yard tractor, #53, in the wash rack area. Sometime between 11:40 a.m. and 11:49 a.m., he apparently suffered a heart attack and, as a result, crashed his vehicle into a Peterbilt tractor, #147, that was parked nearby. His coworkers immediately contacted 911 and CPR was initiated on Employee #1 within minutes. He was transported by paramedics to Providence Medical Center, where at 12:33 p.m. he was pronounced dead. At 12:33 p.m. Medical Center personnel said the cause of death was a heart attack.
