Abstract: At approximately 2:00 p.m. on October 31, 1996, Employee #1 was using a fully-guarded table saw to rip five 13 in. long by 5 in. wide boards. When he had completed the last cut he reached for the thin waste piece, called a tailing, to move it out of the way. The tailing was lying parallel to the blade and spreader, and half of it was still within the guard. As Employee #1 took hold of the tailing, the saw blade, which was still running, grabbed it and pulled his left hand into the blade from behind the hood guard. Three of his fingers were amputated. He was immediately transported to Sacred Heart Hospital, but efforts to reattached his fingers were not successful.
