Abstract: At 1:15 p.m. on August 18, 1994, in San Diego, CA, Employee #1 was working for Aetna Sheet Metal Inc., a company that specializes in the production and installation of sheet metal for roofing systems. They take sheet metal and form and solder it for installation. This product is then installed on various types of roofing systems. Employee #1 had been working independently on a school roof for 1 1/2 hours on a hot day, priming a 4 inch strip of sheet metal that had been installed around the perimeter of the building within 4 inches of the roof edge. He was using a 4 inch roller attached to a 4 foot handle. He applied the primer by walking sideways along the edge of the building. He stated that at the time of the accident he was hot, tired, and in a somewhat dazed condition. He thought he was about 10 feet away from the corner of the building where it made a turn. He said after working that long in the hot sun, it was like looking at a mirage when he looked at the roof. Employee #1 stated he didn't realize that he was falling off the roof until he moved his left foot over the edge and shifted his weight and realized he was not stepping on anything. He fell 11 ft 2 in. from the roof and fractured his neck. The next thing he knew he was lying on the ground and couldn't move. He was hospitalized with a fractured neck. The supervisor was not in the area at the time of the accident. This roof area was not equipped with any perimeter guards and the employees were not offered any type of fall protection. The employer was aware of the need for fall protection for employees working above 7 1/2 feet. Accident related violations were issued: S 1670 (a). Other non-related citations were also issued.
