Abstract: At approximately 8:30 a.m. on March 15, 1996, Employee #1, of Ireland Engineering in Duarte, CA, was using an American 10 in. table saw, model TAS-109, serial #530613, to cut an approximately 3 in. by 3 in. by 2 in. pattern part. A piece of material kicked back and struck Employee #1's safety glasses, seriously injuring his left eye. He was transported to Arcadia Methodist Hospital, where his left eye was removed. The table saw was not guarded and no anti-kickback device was in place. The employer was cited for serious violations of T8CCR 4300(a) and 4300(d).
