Abstract: At approximately 3:00 p.m. on February 22, 1997, Employee #1 was cleaning a Sugarlane juice extractor. The machine was running and plugged into a 120 volt power outlet when Employee #1 lifted the slide gate in front of the rollers. She was reaching in with her left hand to remove the sludge on them when her fingers became caught in the rollers and three of them were amputated. Employee #1 was interviewed at the site on June 16, 1997, and confirmed, through an interpreter, that the machine was running when the accident occurred. It was determined that the microswitch on the machine, which disconnects the power supply to the rollers when the slide door is opened, was not functioning.
