Abstract: Employee #1 was instructed to place two 2 by 4 planks on the floor so that a coworker operating a forklift could place an approximately 2,000 lb stack of wooden planks on top of them. Employee #1 was bent over placing the 2 by 4s when the load fell off the forklift. He jumped back, but his right foot became caught under the planks, crushing his toes. Employee #1 eventually had to have his large toe, second, and third toes surgically amputated.
