Abstract: At approximately 11:30 a.m. on August 23, 1999, Employee #1, of Helix Electric, was unscrewing the 50 lb front cover of an electrical panel in the power room. The panel fell and pinned his left hand against the floor, amputating his middle finger and lacerating his index and ring fingers. Employee #1 was working alone at the time of the accident.
