Abstract: At approximately 10:35 a.m., Employee #1 was using a 12 ft metal extension ladder to work on the spring cable of a garage door. When he was at a height of about 8 to 9 ft, he reached upward to push the door higher. The unsecured ladder slipped out from under Employee #1 and he fell forward, riding the ladder down. His left leg became entangled between the rungs, and he sustained a fractured tibia and fibula, as well as general trauma to his chest.
