Abstract: Employee #1, of Cal-Waste Industries, was using a 4,000 lb Hyster forklift equipped with a 180 degree rotational fork unit to perform his assigned task of emptying small trash bins into a larger one. He approached a small trash bin in the yard and raised it to clear the side of the larger one. He emptied the load by rotating the forklift 180 degrees and then backing up and descending an approximately 8 degree grade used for water run-off. The load was raised, and the forklift wheels were turned to the right when the forklift started falling over on its left side. Employee #1 abandoned the forklift from the left side, and his left ankle became pinned under the forklift chassis.
