Abstract: At approximately 1:10 p.m. on September 20, 2001, Employee #1, a new temporary employee, was removing door molding boards from a conveyor. Employee #1 had been trained not to reach for the boards but let the board come to him and had been warned previously when he did reach for a board on the conveyor. When reaching for another board on the conveyor, Employee #1's right ring finger entered a gap in the conveyor chain and was partially amputated when the drive sprocket came around and pinched it between the sprocket tooth and chain link. Employee #1 was provided first aid and hospitalized for treatment for the flesh amputated from the fingertip.
