Abstract: On July 15, 1999, Employee #1, a pipe fitter, was part of a crew installing a fire sprinkler system at a tilt-up warehouse under construction. Employee #1 was using a MEC scissor lift, model 33SART, serial #10600134, to place hangers about 10 ft apart. As the scissor lift moved, one of the wheels dropped into a 24 in. square, 12 in. deep hole in the otherwise smooth concrete floor. The lift overturned and Employee #1 fell approximately 23 ft, fracturing both legs and sustaining a crushed ankle and a concussion. He was taken by ambulance to Sutter Roseville Medical Center, where he was treated and released on July 21, 1999.
