Abstract: At about 2:30 p.m. on January 22, 2008, Employee#1, Carpet Installer Stokes Flooring, used table saw (Bosch, Model Number 4000, Serial Number 588313762) to cut a 5 in. wide approximately 5 ft long hardwood flooring board. When finished, he grabbed board, lost balance, contacted unguarded blade with left hand, amputating four fingers. The employee was hospitalized, with fingers reattached.
