Abstract: At about 6:50 p.m. on January 13, 1994, Employee #1, of Superior Fast Freight, was unloading a stake bed trailer at the south side of a warehouse with a Hyster forklift when the brakes failed. He lost control of the vehicle, which skidded on several oil puddles on the floor of the warehouse and fell off the dock, a drop of about 4 feet. Employee #1 suffered bruises to his right hip and shoulder. He had been unloading metal bars packed in oil; apparently the oil had leaked, creating puddles on the floor. Employee #1 was interviewed by phone about the accident.
