Abstract: At about 1:15 p.m. on October 4, 1992, according to the plant safety director, Employee #1, of Owens-Brockway, was driving Clark forklift #14 with a pallet load. He placed the load on a turntable in the palletizer area of the plant, backed off, and alighted the lift to adjust the wrap on the turntable when the forklift suddenly started to move toward him. He was pinned between the forklift and the turntable and fractured both ankles. Employee #1 did not set the emergency brake before getting out of the forklift and he did not put the gear in park. According to Employee #1, the emergency brake was not working that day and caused the accident. During the inspection, the employee operating the forklift said that the brakes were functioning O.K.
