Abstract: Employee #1 and a coworker were cleaning out soil that had been sloughing into the bottom of a 130 to 140 ft long by 4 ft deep trench. The coworker was operating a backhoe and Employee #1 was using a shovel when the backhoe's bucket struck a 24 ft long concrete piece embedded in the side of the north wall. The employees continued to dig next to the concrete slab and placed the excavated soil directly above it. The concrete piece broke away from the north bank and fell into the trench, pinning Employee #1's legs. He sustained injuries to his right leg, for which he was hospitalized. The soil was loose and had previously been disrupted by excavation at the same site.
