Abstract: At approximately 1:00 p.m. on January 22, 1996, Employee #1 was working from an extension ladder of Albertson's warehouse in North Salt Lake, UT. He was trying to remove a metal post when the bottom of the ladder slid away from the wall. Employee #1 fell head first approximately 17 ft, landing on a concrete floor. He suffered head and back injuries. Employee #1 was treated by Davis County paramedics, then transported by Life Flight helicopter to the hospital.
