Abstract: At approximately 8:45 a.m. on December 17, 1995, Employee #1, a worker at a glass container manufacturing plant in Vernon, CA, was notified that overhead conveyor line C-2 was not working properly. Without turning off the conveyor, he climbed a ladder and attempted to repair it. He was installing rubber bands on the conveyor shaft when his right middle finger was pulled in by a band. Employee #1 screamed, and a nearby coworker turned off the conveyor. He suffered a dislocated and compound fracture of his finger. This accident resulted from failure to de-energize or disengage the power source. The employer, Owens-Brockway, was cited for a general violation of T8CCR 3314(a).
