Abstract: Employee #1, an experienced heavy equipment mechanic for Advanco Constructors, Upland, CA, was servicing a Lorrain rough-terrain crane, model LRT 1600M, on a construction site. The boom was dropped so that the tip was chest-high to the employee, and the hook and block were resting on the ground. He tried to grease the point sheave, but the xerc fitting was not in a position to be greased. The sheave could not be turned because the load rope was bound up. He signaled for the operator to pay the rope out, and pulled down on the rope so it would spool out. When the operator accidentally pulled the rope in, instead of paying it out, Employee #1's right little finger was pulled into the sheave, amputating a portion of the last phalange.
